htaccess rewrite, add new top level folder to existing link paths - dynamically

I've searched about and cannot find specifically what I need and wondered if anyone could provide an example.

I have an old site with URLS like:

http://ift.tt/1LTV3DW

and need to redirect dynamically to:

http://ift.tt/1RBNHSj

Of course, there are a lot of regionname/countyname combinations so need to use pattern matching of sorts!

Additionally, I then need to redirect:

http://ift.tt/1LTV3DZ

to:

http://ift.tt/1RBNI8x

...since my new site allows listings in multiple counties!

I've found similar examples but not quite what I need and this really isnt my area of expertise. I'm sure I'm making a lot more work out of this than I need to :/
